Three SAPS members, two awaiting-trial prisoners and civilian driver die in R524 Levubu Road collision
Six people have died following a head-on collision involving a South African Police Service vehicle and a civilian bakkie on the R524 Levubu Road in Limpopo’s Vhembe District.
The crash happened on Tuesday morning, 6 October 2026, when a police vehicle from Tshilwavhusiku Police Station, which was transporting three awaiting-trial prisoners to a correctional facility in Thohoyandou, collided with a white Mahindra bakkie belonging to Tshisimani Water.
Six people confirmed dead
According to the South African Police Service, emergency medical services initially certified five people dead at the scene.
A third SAPS member, who had sustained serious injuries in the collision, was transported to a local hospital for medical treatment but later succumbed to those injuries, bringing the confirmed death toll to six.
The deceased comprise three police members – one male and two females – two male awaiting-trial prisoners and the male driver of the civilian bakkie.
The third awaiting-trial prisoner survived the crash but sustained serious injuries. He was taken to hospital under police guard.
The identities of those who died have not yet been released. Police said the names will only be made public after the necessary procedures have been completed and their next of kin notified.
Police investigation underway
Limpopo police spokesperson Brigadier Hlulani Mashaba confirmed that a case of culpable homicide has been opened at Makhado Police Station.
“The circumstances surrounding the collision are being investigated,” SAPS said in its statement.
Relevant police units and senior management attended the scene as emergency personnel dealt with the aftermath of the crash.
At this stage, police have not publicly attributed blame to either driver, and the circumstances that led to the head-on collision remain the subject of the investigation.
The opening of a culpable homicide case means investigators will examine the circumstances surrounding the deaths and determine whether any criminal negligence or other unlawful conduct can be established.
Police commissioner sends condolences
Limpopo Provincial Commissioner Lieutenant General Jan Petrus Scheepers expressed his condolences to the families, colleagues and loved ones of those who died.
“We extend our sincere condolences to all the bereaved families, colleagues and loved ones of those who lost their lives in this tragic incident. We also wish the injured a speedy recovery,” Scheepers said.
The deaths are particularly significant for SAPS as three of its members were among those killed while carrying out police duties involving the transportation of awaiting-trial prisoners.
Prisoner was being transported to correctional facility
The SAPS vehicle involved in the crash was transporting three awaiting-trial prisoners from Tshilwavhusiku Police Station to a correctional facility in Thohoyandou when the collision occurred.
Two of the prisoners died in the crash, while the third was seriously injured and remains under police guard while receiving medical treatment.
The incident therefore involved not only the loss of three police officers and a civilian motorist, but also two people who were still awaiting the conclusion of their respective criminal proceedings.
Awaiting-trial status does not amount to a conviction, as a person remains legally presumed innocent until proven guilty in court.
Investigation to establish what happened
Police have not yet provided details about what caused the vehicles to collide head-on or whether factors such as speed, road conditions, vehicle condition or driver conduct played a role.
Those issues will form part of the investigation into the collision.
The R524 is an important road linking communities in the Vhembe region, and serious crashes on the route have previously raised concerns around road safety. However, no specific road condition has been identified by police as a cause of Tuesday’s collision.
Further information is expected as investigators continue with the case and authorities complete the process of notifying the families of the deceased.
